Welcome to SureCare WA (“we”, “our”, “us”). These Terms & Conditions govern your use of our website www.surecarewa.com.au and the services we provide as a registered NDIS service provider in Western Australia.

By accessing our website or engaging our services, you agree to comply with these Terms.

 

2. About Our Services

SureCare WA provides disability support services in accordance with the National Disability Insurance Scheme (NDIS) guidelines.

Our services may include (but are not limited to):

  • Personal care assistance
  • Community participation
  • Supported independent living (SIL)
  • Transport assistance
  • Respite care
  • Support coordination

All services are delivered in line with:

  • NDIS Practice Standards
  • NDIS Code of Conduct
  • Relevant Western Australian laws

 

3. Eligibility for Services

To access our services, you must:

  • Be an approved participant under the NDIS
  • Have a valid NDIS plan
  • Agree to a Service Agreement with SureCare WA

 

4. Service Agreements

Before services commence:

  • A written Service Agreement will be provided
  • This will outline:
    • Type of supports
    • Pricing (as per NDIS Price Guide)
    • Cancellation policy
    • Responsibilities of both parties

 

5. Fees and Payments

  • Fees are charged in accordance with the NDIS Pricing Arrangements
  • Payments may be:
    • NDIA-managed
    • Plan-managed
    • Self-managed

You agree to:

  • Pay any applicable fees not covered by NDIS
  • Provide accurate billing information

 

6. Cancellation Policy

Participants must provide:

  • Minimum 24–48 hours’ notice for cancellations

Failure to do so may result in:

  • Charges in line with NDIS cancellation rules

 

7. Participant Responsibilities

You agree to:

  • Treat staff with respect
  • Provide a safe environment for service delivery
  • Inform us of any risks or changes in condition
  • Comply with agreed schedules

 

8. Our Responsibilities

SureCare WA commits to:

  • Delivering high-quality, person-centred supports
  • Respecting your dignity, privacy, and independence
  • Employing trained and qualified staff
  • Maintaining compliance with NDIS regulations

 

9. Privacy and Confidentiality

We handle your personal information in accordance with the Privacy Act 1988.

We will:

  • Protect your personal data
  • Only use information for service delivery
  • Not disclose information without consent (unless legally required)

 

10. Website Use

By using our website, you agree:

  • Not to misuse or attempt to hack the site
  • Not to copy or reproduce content without permission
  • That content is for general information only

We do not guarantee:

Either party may terminate services:

  • In accordance with the Service Agreement

  • There is a risk to staff safety
  • Terms are breached

We may update these Terms & Conditions at any time. Updates will be posted on this page.

 

15. Governing Law

These Terms are governed by the laws of Western Australia.
